Carlo C. DiClemente, PhD, ABPP
Dr. Carlo DiClemente is an emeritus professor of psychology at the University of Maryland Baltimore County and Director of the Maryland Tobacco Resource Center at the University of Maryland Baltimore County (UMBC). He is co-developer of the Transtheoretical Model of behavior change, and author of numerous scientific publications on motivation and behavior change with various health and addictive behaviors. He has published extensively on smoking cessation, brief motivational interventions, and initiation and recovery from addictions. He is the author of Addiction and Change: How Addictions Develop and Addicted People Recover. He has also co-authored several professional books, The Transtheoretical Model, Substance Abuse Treatment and the Stages of Change, and Group Treatment for Substance Abuse: A Stages of Change Therapy Manual and a self-help book, Changing for Good.
Reba Mathern-Jacobson, MSW, leads American Lung Association’s behavioral health projects in MN and ND. She coordinates the Lung Mind Alliance which involves statewide partners to implement strategies to reduce the disparities and impact commercial tobacco use has on people with mental illness and/or substance use disorders. She has over 20 years of experience living and working within behavioral health settings, doing program development and policy.
Pat McKone, Senior Director –Public Policy and Advocacy - American Lung Association, has worked with lung health programming for over forty-five years. Ms. McKone’s work has included tobacco cessation programs for adults and youth, limiting youth access to tobacco, public and school-based education, raising awareness of the impact of tobacco in those with mental illness and/or substance abuse disorders, as well as advocacy around limiting exposure to secondhand smoke, smoke free housing, point of sale, and e cigarettes.
Pat was instrumental in founding the Lung Mind Alliance, a statewide partnership between mental health, substance abuse, and tobacco control leaders in Minnesota working together over the past eight years to address disparities related to commercial tobacco in these populations.
Ms. McKone was the 2017 recipient of the American Lung Association’s Distinguished Professional Service Award and in 2018 was recognized as part of the Blue Cross Trailblazer award.
Ms. Moncrief is the Director of the Maryland Department of Health, Center for Tobacco Prevention and Control. She has been in state service and with the Center for over 15 years. As part of her role, Ms. Moncrief oversees the state tobacco control program, which includes statewide public health, local public health, cessation and health systems, surveillance and evaluation, and enforcement. Since May of 2020, she has also served as the State Lead for Pediatric COVID Vaccine Initiatives, working closely with providers across the state to prepare for vaccine administration for residents under the age of 18.
Ms. Moncrief holds her Master of Health Science from the Johns Hopkins Bloomberg School of Public Health, a Bachelor of Science degree in biology from The Pennsylvania State University and is a Certified Health Education Specialist.

AMA PRA Category 1 CreditTM is acceptable to meeting the CE requirements for the California Board of Psychology. Providers in other states should check with their state boards for acceptance of CME credit.
